Prescription Medication Policy
Prescription medication must be prescribed by your child’s licensed Arizona healthcare provider and filled by a licensed pharmacy. No Prescriptions from outside of the United States will be accepted.
• Prescription medications must be given to the health office staff with a clear pharmacy label affixed to the bottle with the child’s name, drug name, dosage, instructions for use, date prescribed, pharmacy name, and expiration date. Bottles missing this needed information will not be accepted.
• Medications prescribed to be taken one (1) two (2) and three (3) times a day are not routinely given at school. Exceptions may be made (ADHD medications) if the nurse discusses the need with the parent/guardian or licensed healthcare provider and they find this is necessary.
• A Consent to Dispense form must be filled out and signed by the child’s parent/guardian, for prescription medication to be given.
• It is the responsibility of the parent/guardian to update the health office if there are changes in medication dosage and/or frequency. A new Consent to Dispense form must be completed and signed by the child’s parent/guardian for any changes to prescription medication (dosage, timing, etc.), including each new school year.
• Medication must be given to the Health Office staff and is not to be left with the front officepersonnel unless otherwise discussed with health office staff.
• Health Office staff may count medication with parent/guardian or secondary school staff to verify inventory count received. Only a 30-day supply is to be received in office.
• Parent/guardian is responsible for providing prescription medications for overnight field trips with all appropriate paperwork. Medications must be given to field trip lead by the parent/guardian and picked up upon return by the parent/guardian.
• All unused or expired medications are to be picked up by parent/guardian at the end of the prescribed date or at the end of the school year. Medications not picked up within a week of being notified will be disposed of in accordance with federal guidelines.
• It is the responsibility of the parent/guardian to cut medications in half. If medications are to be crushed, a doctor’s order is needed, parents/guardians are to provide the office with a personal pill crusher for their child and other needed supplies.
• Narcotic pain medication, CBD oil and medical marijuana will not be given in the general education setting at school for the safety of all students. (ARS§15-108)
